With as much torque as they make we could probably gear the crap out of them and make up for the lack of RPM with multiple gear transmissions. Like all the 3 speeds that are already out there. We have messed around with a few 4-strokes in 1/5th scalers and there just isn't anything that can handle the stress at least in the large scale rigs. Most of what is available has plastic valvetrain and power output is extremely weak. But yeah if someone would make somthing durable enough it would be a good thing.

I've seen a couple 1/10 scale project trucks around with four-stroke airplane engines. Slower than molasses, but sounded so much better than the angry bee sounds of two strokes.
